INSTRUCTIONS Download the attached file to your computer. (It will probably save in “Downloads”)
You need to have the programme “Adobe Acrobat Reader” (it’s a free download.) Double click the file “FamilyInfo-complete.pdf” to open the file.
When it’s open, down the right hand side is a list of options. Click on “Fill&Sign”. A new window appears.
Click the button “Fill and Sign” All the input boxes are now ‘fillable’. As you click on each box, it becomes highlighted in blue. Click inside the blue box and the box will then say “Type Text Here”
(When completed, you should ‘save’ the file with a different name than the original, so that you can use the ‘blank’ form again if necessary.)
